Cubs Fact:
Harry Caray and Ryne Sandberg of the Cubs both bowed out on September 21, 1997. Sandberg's last home game coincided with Caray's last broadcast at home, as he passed away early in 1998.

I do remember that Sandberg was replaced with a pinch runner in that game and all the players met him at the dugout to shake hands so i would imagine everyone knew it was his last home game.
